Team Manager Information

Team Managers,

For each player and coach, please complete a USA Hockey Code of Conduct Form and a USA Hockey Consent to Treat Form.  Please have at least one parent (both would be great, if available) complete a USA Hockey Code of Conduct Form.
Here are the pdfs of these two forms:  USA Hockey Code of Conduct Form  www.richmondroyals.com/files/2008-2009_code_of_conduct.pdf
USA Hockey Consent to Treat Form www.richmondroyals.com/files/2008-2009_consent_to_treat.pdf
Please place these forms in your team binder in the appropriate sections.

In addition, please have your players start registering with USA Hockey for the 2008-2009 season.  All players must be registered with USA Hockey.  Please ask your parents to send you a copy of the registration form for you to forward as a group to Susan Varner www.usahockeyregistration.com/ . The parents may send a copy (preferably via email) to Susan but, if they do, please make sure they notify you as Team Manager and send you a copy too.

Also, Team Managers need to send an updated roster to Susan Varner complete with players names, jersey numbers, birthdates, addresses and telephone numbers.  Please email to Susan at sdvarner@yahoo.com.
